The purpose of this form is to inform you about the activity and seek your support and permission for your child to participate. All NSA activities are led by NSA Leaders who have been Safety Checked and are organized according to NSA Policy to ensure the maximum safety for each participant. Please note that a form signed by you along with any required payments is needed before your child can begin participation.

1. Cash or Cheque at on site Registration
